Vancomycin Injection 500mg

Vancomycin (as HCl) 500mg/vial, 1g/vial; lyophilized pwd for IV infusion after reconstitution and dilution. Indications for Vancomycin Injection: Serious or severe infections due to susceptible methicillin-resistant (beta-lactam-resistant) staphylococci in penicillin-allergic patients; patients who cannot receive or have failed to respond to other drugs, including penicillins and cephalosporins; and for infections due to vancomycin-susceptible organisms resistant to other antimicrobials. Staphylococcal endocarditis, including septicemia, and infections of the lower respiratory tract, bone, and skin and skin structure. Streptococcal endocarditis due to S. viridans or S. bovis as monotherapy or in combination with an aminoglycoside. Adult: Give by intermittent IV infusion over at least 60 minutes or at a rate of 10mg/min or less, whichever is longer. 2g/day in divided doses (500mg every 6 hours or 1g every 12 hours). Renal impairment: adjust or reduce dose, obesity; see literature. Children: month: initially 15mg/kg, then 10mg/kg every 12 hours (for neonates in the 1st week of life) and every 8 hours thereafter (up to 1 month of age). >1 month: 10mg/kg per dose every 6 hours. Renal impairment: adjust or reduce dose, obesity; see literature. Warnings/Precautions: Not for prophylaxis of endophthalmitis. Renal insufficiency. Monitor hearing, blood, and renal function. Monitor vancomycin serum concentrations closely. Elderly. Debilitated. Pregnancy. Nursing mothers. Interactions: Monitor with other ototoxic, nephrotoxic, neurotoxic drugs (eg, aminoglycosides, amphotericin B, bacitracin, polymyxin B, colistin, viomycin, cisplatin). Erythema, histamine-like flushing, anaphylactoid reactions may occur with concomitant anesthetic agents. Pharmacological Class: Antibiotic. Adverse Reactions: Infusion-related events, nausea, drug fever, rash (may be serious, eg, Stevens-Johnson syndrome), ototoxicity, nephrotoxicity, thrombophlebitis, blood dyscrasias; rare: interstitial nephritis, vertigo.
