Lenamed(Lenalidomide) is used for the treatment of:

1. Multiple Myeloma in combination with dexamethasone
2. Myelodysplastic Syndromes with deletion 5q abnormality
3. Mantle Cell Lymphoma in patients who have relapsed or progressed
4. Follicular Lymphoma & Marginal Zone Lymphoma relapsed/refractory
5. Chronic Lymphocytic Leukemia in clinical settings

Mechanism of Action

• Lenalidomide is athalidomide derivativewith enhanced potency.
• It:
  • Inhibits angiogenesis(formation of new blood vessels in tumors)
  • Stimulates T cells & NK cells(immune response)
  • Inhibits pro-inflammatory cytokines
  • Induces apoptosis(programmed cell death) of cancer cells

Dosage & Administration

• Multiple Myeloma:Usually 25mg daily for 21 days in a 28-day cycle but10mgmay be used in dose reductions
• MDS:10mg once daily continuously
• Swallow capsules whole;do not open, crush, chew
• Prescribed intreatment cyclesbased on indication & tolerance

Common Side Effects

• Anemia, Neutropenia, Thrombocytopenia
• Fatigue, weakness
• Nausea, constipation or diarrhea
• Rash, Itching
• Dizziness, headache
• Muscle cramps or joint pain
• Increased risk of blood clots (DVT/PE)

Monitoring

• CBC
• KFT & LFT
• Pregnancy testingbefore & during use (in females of childbearing potential)
• Monitor forsigns of thrombosis
• Thyroid function tests

Precautions

• Pregnancy Category XSeverely teratogenic
  • Must followREMS program(US) or equivalent in other countries
  • Requires2 forms of contraception& regular pregnancy testing
• Caution inrenal impairment dose adjustments required
• Increased risk of2ndry malignancies
• Avoid driving duringdizziness or drowsiness

Drug Interactions

• Dexamethasone used in combination but increases risk of blood clots
• May interact withwarfarin(monitor INR closely)
• Concomitant use withstrong CYP3A inhibitors/inducersmay alter drug levels

Tips for Patients

• Do not share medication even 1 capsule can cause birth defects
• Follow pregnancy prevention guidelines strictly
• Reportunusual bleeding, bruising, or infections
• Drink plenty of fluids unless restricted
• Avoid activitiesthat increase bleeding risk
• Take medicationat the same time each day,if missed
