Lenvatinib Capsule 10mg - Origin: India

Letinib(Lenvatinib) is atyrosine kinase inhibitor (TKI)used in the treatment of:

• Thyroid cancer(radioactive iodine-refractory differentiated thyroid cancer)
• Renal cell carcinoma(advanced kidney cancer, often in combination with everolimus)
• Hepatocellular carcinoma(HCC, first-line for unresectable liver cancer)
• Endometrial carcinoma(advanced, in combination with pembrolizumab)

Mechanism of Action

Lenvatinib blocks multiple receptor tyrosine kinases (RTKs), including:

• VEGFR1-3, FGFR1-4, PDGFR, RET, & KIT
• This inhibitsangiogenesis(blood supply to tumor) &tumor cell proliferation
• Leads to tumor shrinkage & slowed progression

Dosage & Administration

• Thyroid cancer: 24mg orally once daily
• HCC: 8mg or 12mg once daily depending on body weight
• Kidney/Endometrial cancer: 18mg once daily (with everolimus or pembrolizumab)
• Take at thesame time each day,with or without food
• Swallow capsules whole with water
• Dose adjustments may be required for side effects, liver/kidney issues

Common Side Effects

• Hypertension
• Diarrhea, nausea, vomiting
• Fatigue, weakness
• Decreased appetite, weight loss
• Hand-Foot syndrome
• Protein in urine
• Abnormal LFT
• Bleeding or thromboembolic events (rare but serious)

Monitoring

• Blood pressureregularly (especially in first weeks)
• KFT&LFT
• Thyroid Tests(if applicable)
• Urinalysis forproteinuria
• Electrolytes, ECG (QT prolongation risk)
• Monitor for signs ofbleeding,GI perforation, orheart issues

Precautions

• Pregnancy Category D causes fetal harm; contraception required during & after treatment
• Caution in patients withcardiovascular disease,uncontrolled hypertension, orliver impairment
• Avoid use with strongCYP3A4 inducers/inhibitors
• Can impair wound healing stop before surgery & restart after proper healing
• May increase risk ofbleeding,perforation,fistula formation

Drug Interactions

• CYP3A4 inhibitors(e.g, ketoconazole) may increase lenvatinib levels
• CYP3A4 inducers(e.g, rifampin, phenytoin) may reduce effectiveness
• Antihypertensives use cautiously; monitor BP
• Anticoagulants monitor bleeding risk closely

Tips for Patients

• Use daily at the same time with water
• Monitor & control blood pressure
• Stay hydrated & eat small frequent meals
• Inform doctor of any signs ofbleeding,severe diarrhoea, orabdominal pain
• Use effective contraception during & for at least 1 month after last dose
• Do not skip doses or double up if missed take next dose as scheduled
