Mode of action of Azithromycin

The macrolides antibiotics inhibit bacterial protein synthesis via binding to the 50s ribosomal subunit of susceptible bacteria. They may be bacteriostatic or bactericidal depending on the organism and the antibiotic concentration.

Antibacterial spectrum

Erythromycin is active against gram-positive organism and gram-negative organism. The newer macrolides have similar activity to erythromycin but Azithromycin has better gram-negative activity than erythromycin and roxithromycin.. Azithromycin is more potent among all the macrolides for its activity against atypical pathogens.

Phagocytic mechanism of action

The significant uptake of Azithromycin by phagocytes also suggest that these cells may play a role in the transport of Azithromycin to tissues sites of infection and inflammation

Step 1 = phagocyte absorb Azithromycin in circulation and tissue

Step 2 = WBCs migrate to sites of infection in tissues

Dosage :

Typhoid 1000mg stat daily till defervascence, followed by500mg daily for 15 days

Sexually transmitted disease -1000mg stat

Syphilis 500mg OD for 10 days

Gonococcal Gonorrohea 2gm stat

or 500mg OD on day 1 followed by 250mg daily for 4 days.

In children 10mg / Kg on day 1 followed by 5mg/Kg for 4 days.

or 10mg OD for 3 days.
