No dangerous goods can be loaded on SAL's vessels or vessels freighted by SAL without prior acceptance of SAL Dangerous Cargo Office who will issue acceptance number. Acceptance of hazardous cargoes does not relieve shipper and booking agent of their obligations and agent will be held fully responsible for any consequences arising out of their negligence, or negligence of any person working for them directly or indirectly.

* CONSECUTIVE HAZARDOUS REQUEST NUMBER
* VESSEL NAME
* VOYAGE NUMBER
* PORT OF LOADING
* PORT OF DISCHARGE
* FINAL DESTINATION
* PORT OF TRANSSHIPMENT (if POD is different from FD)
* ROUTING (if POD different from FD and Port of origin is different from POL)
* TYPE AND SIZE OF CONTAINER
* NUMBER AND KIND OF OUTER PACKING
