The instrument is highly useful as Diabetes is recognized as being a prominent cause of foot ulceration and infection, followed by non-traumatic lower-limb amputation. This happens because of the loss of protective sensation due to peripheral neuropathy for which there is no preventive cure. Hence, the best strategy is to encourage early detection of neuropathy that allows secondary prevention measures can be implemented.
The instrument has a standardized filament (Semmes- Weintein nylon filament), which is pressed against the foot. On bending, the tip of the bending exerts a pressure of 10 grams. A patient is at risk of developing a neuropathic ulcer when he loses too much sensation. The instrument measures the same as in such cases the patient cannot feel the monofilament at specified areas on the foot.
