The Full Name, Address and Nationality of any person who is authorized to assign or license the rights in the Copyright.


6 copies of the work.


Declaration should be signed by the Author of the Work, if the Applicant is different from the Author.


Application Forms and Power of Attorney to be signed by the Applicant.


We shall provide you with the duly filled in Authorization Form after receiving the details or you may copy the same from the attached Authorization Form for Copyright Registration.
